Insurance

All co-payments are due at the time of the visit.

We accept VISA, MasterCard, American Express, Discover, cash and checks.

Our doctors participate with the insurance plans listed below, but are happy to work with other insurance plans that accept out-of-network benefits

Blue Cross/Blue Shield, Medicare, Oxford, Workers Comp, No Fault

If you have another type of insurance, we will submit to your insurance company for you. However, you may receive a bill if there are charges that are not covered.

If you need a referral, it is your responsibility to check with your insurance company and bring it with you to the visit.

*It may take at least 48 hours for our office to obtain MRI authorization. We will contact you as soon as we get the approval. All insurances must be called except for MEDICARE.

Medical Records

Medical records include documentation of your medical care delivered within our practice including your doctor, physician assistant, hand and/or physical therapist, MRI and radiology personnel.

The State of New York requires that we receive formal consent to release medical records in order to protect your health information. To release your medical records, simply fax or mail the consent form below to our office. Please do not e-mail.
